| Useful Information NEWLY REGISTERED PATIENTS All new patients joining our practice will be asked to arrange an appointment with either a nurse or doctor. This allows us to assess your medical needs and to record accurately any medication you require. It is an ideal opportunity for new patients to become familiar with the practice. CARERS REGISTER The practice has a register of patients who care for someone who is ill, frail, disabled or mentally ill. The aim is to provide additional support if necessary and where possible. If you are a carer and registered with this practice, would you please complete one of the Carers Questionnaires available from reception. CHANGE OF ADDRESS We are required by the Health Board to notify them immediately of any change of name and/or address of our patients. Please inform the surgery as soon as possible of any changes you may have to the above details. PATIENTS AGED 75 AND OVER In the past, our Health Visitors have performed annual home visits to patients in the 75+ age group. Many patients in this age group are, happily, very fit and active. The health visitor service is now targeted to those patients who have chronic illnesses or are housebound. We would advise all patients of 75 and over to have an annual health check and would suggest that you should make an appointment to see one of our nurses. If you cannot come to the surgery, we will be pleased to arrange for the health visitor or district nurse to call. WORK AND SICKNESS Self-certification with form SC1 is all that is required for illnesses lasting six days or less. This form is available at the DSS, chemists and our reception desk. After six days you must consult a doctor or, under certain circumstances, Sister Cooper for further certification. PRIVATE MEDICALS Insurance medicals and non-NHS examinations are undertaken by mutual arrangement with the GP of your choice (eg HGV medicals, sports medicals). A fee will be charged in accordance with BMA rates. |
